The present study aimed to investigate the effect of strength and balance exercises on reducing bone pain and improving gait pattern in elderly women. This research was conducted using a quasi-experimental design with a pretest–posttest format and a control group. The statistical population consisted of elderly women aged 60 to 75 years in Semnan city, from whom 30 participants were selected through convenience sampling and randomly assigned to either an exercise group (n = 15) or a control group (n= 17). Pain intensity was measured using the Visual Analog Scale (VAS), and gait pattern was assessed through temporal and spatial gait parameters. The exercise group participated in an 8-week training program consisting of three sessions per week, each lasting 45 to 60 minutes, including lower-limb strength training and balance exercises. The control group did not engage in any specific exercise program during this period. Data were analyzed using paired-samples t-tests and analysis of covariance at a significance level of 0.05. The results indicated that after the intervention, pain intensity in the exercise group significantly decreased. Furthermore, gait parameters including step length, walking speed, and dynamic balance significantly improved (p < 0.05). In contrast, no significant changes were observed in the control group. Between-group analysis also revealed significant differences between the two groups in pain and gait-related variables.
